What causes tooth resorption?
There is not always one identifiable cause. Reported risk factors and associations include dental trauma, inflammation or infection of the pulp, previous orthodontic movement, pressure from an unerupted tooth or another lesion, and some dental procedures. External cervical resorption can occur near the gum line and may be difficult to see during a routine visual examination.
Having one of these risk factors does not mean resorption will occur. Likewise, a person may be diagnosed without remembering any injury. The clinical history, examination and imaging must be interpreted together.

Assessment usually begins with the history of trauma, orthodontic care, bleaching, previous root canal treatment and recent symptoms. The dentist may inspect the tooth, probe the gum line, test pulp responses, check mobility and gently assess tenderness.
Dental radiographs are central to diagnosis. Taking images from more than one angle may help determine whether a defect is internal or external. Cone-beam computed tomography (CBCT) can provide three-dimensional information when conventional images do not show the location or extent clearly. CBCT is not automatically required for every patient; the expected diagnostic benefit should justify the radiation exposure.

Treatment options
Treatment is individualized. The dentist considers the resorption type, activity, size, location, pulp status, remaining tooth structure and whether the tooth can be predictably restored.
Internal resorption
For active internal inflammatory resorption in a restorable permanent tooth, root canal treatment is commonly used to remove inflamed tissue and disinfect and seal the canal system. Severely damaged or non-restorable teeth may need extraction, but that decision should follow a full assessment.

Can tooth resorption be stopped?
Lost tooth tissue does not grow back in the same way as skin. However, timely management may arrest or control some forms of resorption and preserve the tooth. Prognosis is generally better when the lesion is detected before extensive structural damage or infection develops.
People with a history of dental trauma should keep follow-up appointments even if the tooth feels normal. Orthodontic patients should also attend recommended reviews so root changes can be assessed when clinically indicated.
When should you see a dentist?
Arrange a dental evaluation if a tooth changes colour, becomes loose, develops a defect near the gum line, or remains tender after trauma. Seek urgent care for facial swelling, fever with dental symptoms, pus drainage, rapidly worsening pain, or difficulty opening the mouth. Difficulty breathing or swallowing, spreading facial or neck swelling, or swelling near the eye requires emergency medical assessment.

Is tooth resorption the same as tooth decay?
No. Tooth decay is caused by acid produced by plaque bacteria acting on the tooth surface. Resorption involves the body’s own resorbing cells removing dental tissue. The two conditions can sometimes resemble one another on examination.
Can tooth resorption happen after braces?
Some shortening of roots can occur during orthodontic treatment, but the risk and severity vary. Dentists and orthodontists monitor clinically important changes and adjust treatment when needed.
Does internal resorption always hurt?
